Physical fitness and assumption of risk I declare that I am physically and mentally fit to participate in a full-contact karate event, that I have no medical condition that contraindicates participation, that I have not been diagnosed with a concussion in the last 30 days, and that I am not under the influence of alcohol or drugs. I understand Kyokushin karate involves inherent risks of injury and I accept those risks voluntarily. Liability waiver I release the organizers, officials, medical staff, instructors, sponsors, and venue operators from civil liability for injuries arising from my participation, except in cases of proven willful misconduct or gross negligence. Medical authorization I authorize first aid and transfer to a medical facility if needed. Any further medical costs are my responsibility. I have medical insurance or accept full responsibility for medical expenses. Privacy notice Personal data will be used for event registration, sporting classification, emergency medical care, and results publication. Image authorization I authorize capture and use of my image, voice, and name in photos, video, livestreams, and institutional publications for sporting and promotional purposes, without compensation. Force majeure The organizing committee may modify, suspend, or cancel the event for reasons beyond its control. Travel and lodging costs will not be reimbursed; registration may be rescheduled or partially refunded as applicable. Rules acceptance I have read and accept the tournament rules. I will respect officials' decisions as final and maintain respectful conduct. Parental / guardian note If registering a minor, the submitting adult confirms parental or legal guardian approval for participation and image use.
